Community Paramedicine
Community Paramedicine is a non-emergency health care program that helps eligible patients receive care at home or in the community.
- Who may be eligible? Homebound patients, patients with mobility barriers, patients with frailty, and patients with cognitive barriers.
- How do I get referred? Referrals must come from a health care provider or allied health partner.
- Can I refer myself? You can contact the office to ask questions. In most cases, enrolment requires a referral.
- Does CP replace my doctor? No. Community Paramedicine supports your existing care team.
When to Call
Call Community Paramedicine if the patient is already enrolled and there is a non-emergency health concern where a home visit may help prevent deterioration or avoid a 911 call.
